peaceful tuesday~

early in the morning cum in new pt ,almost all accident again..
the curiosity not more like last time dy..
then went to resus then saw ytd pt that free fall from 4m high.(construction)
ytd 3 pt cum in constantly n 1 die dy~
thr's 2 more left..
luckily 2 more still survive~
then 1st pt~
one chinese uncle cum in complaining gastric with hx of tuberculosis n a smoker for past 40yearss~
pt also complaining cannot sleep whole day,phlegm is black in color,and his weight is 49kg.
according to him admitted to ward last 2 months then the doct had changed the tb medication..
the new tb med cause gastric ...
i'm so stupid enough that for first i dunno that the pt is tb patient~
then i'm nt wearing any protection n jz going to icu ward..
after staying for few minutes the doct only told me this is tb pt.cause he jz complaining gastric.
so..the "wao" thing about is the malay doct can really speak fluency chinese ..haha

So.. wat act is TB?

Tuberculosis (TB) is a bacterial infection caused by a germ called Mycobacterium tuberculosis. The bacteria usually attack the lungs, but they can also damage other parts of the body. TB spreads through the air when a person with TB of the lungs or throat coughs, sneezes or talks. If you have been exposed, you should go to your doctor for tests. You are more likely to get TB if you have a weak immune system.
Symptoms
  • A bad cough that lasts 3 weeks or longer
  • Weight loss
  • Coughing up blood or mucus
  • Weakness or fatigue
  • Fever and chills
  • Night sweats
Risk Factor :
The following people are at higher risk for active TB:
  • Elderly
  • Infants
  • People with weakened immune systems, for example due to AIDSchemotherapy, diabetes, or certain medications
Your risk of contracting TB increases if you:
  • Are in frequent contact with people who have TB
  • Have poor nutrition
  • Live in crowded or unsanitary living conditions
The following factors may increase the rate of TB infection in a population:
  • Increase in HIV infections
  • Increase in number of homeless people (poor environment and nutrition)
  • The appearance of drug-resistant strains of Tb
    diagnosis of TB involves skin testschest X-rays, sputum analysis (smear and culture), andPCR tests to detect the genetic material of the causative bacteria.

    Treatment

    The goal of treatment is to cure the infection with drugs that fight the TB bacteria. Treatment of active pulmonary TB will always involve a combination of many drugs (usually four drugs). All of the drugs are continued until lab tests show which medicines work best.
    The most commonly used drugs include:
    • Isoniazid
    • Rifampin
    • Pyrazinamide
    • Ethambutol
    Other drugs that may be used to treat TB include:
    • Amikacin
    • Ethionamide
    • Moxifloxacin
    • Para-aminosalicylic acid

    after few hours came in new pt~TB again.. pengsan lo~! this time quickly wear mask..==

    then... thr's another CCF pt~congestive cardiac failure~
    the Pt has pitting oedema(leg),SOB..
